RFA: Perinatal and Infant Community Health Collaboratives

On July 28th, the New York State Department of Health (DOH) released a Request for Applications (RFA) to support the implementation of the Perinatal and Infant Community Health Collaboratives (PICHC) initiative. Through this program, DOH will award a total of $70 million over five years to approximately 25 programs statewide to improve perinatal and infant health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. Programs will be responsible for developing, implementing, and coordinating:

  • Individual-level strategies to address perinatal and infant health outcomes and behaviors, including the use of community health workers for outreach and engagement; and 
  • Community-level strategies to address the social determinants which affect health outcomes.

PICHC activities will primarily focus on Medicaid-eligible prenatal, postpartum, and interconception individuals and their families. Eligible applicants are not-for-profit 501(c)(3) organizations, including community-based organizations and Article 28 health care facilities.
