On September 4th, the New York State Department of Health (DOH) released a Request for Applications (RFA) for the local administration of Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) services that are part of the State’s Home and Community Based Services (HCBS) 1915(c) waiver programs. Over $45 million in total funding will be available through this RFA for the local administration of NHTD and TBI services through Regional Resource Development Centers (RRDCs) to be operated by not-for-profit agencies in 10 regions throughout the state.
The NHTD and TBI waiver programs are both 1915(c) waiver models that are designed to reduce unnecessary institutionalizations in a discrete population requiring a nursing home level of care through:
- Transition: Assisting eligible individuals currently living in nursing homes to move to appropriate community-based settings; and
- Diversion: Preventing institutional facility placements by providing community-based services and supported to eligible individuals.
