OMH Releases RFA to Support Specialty Mental Health Care Management Agencies with Critical Transitions
On November 21st, the New York State (NYS) Office of Mental Health (OMH) released a Request for Applications (RFA) to support access to Health Home Plus (HH+) services statewide. Through this opportunity, OMH will provide up to $2.5 million in total funding over one year across 63 awards. There will be 23 awards in New York City and 10 awards in Long Island.
Funding is specifically targeted for OMH-designated Specialty Mental Health Care Management Agencies (SMH CMAs) to develop formal pathways with hospitals, homeless shelters, correctional/criminal justice agencies, and other key providers serving the HH+ population. Such pathways will be designed to receive referrals for HH+ services and allow for timely and effective transitional care coordination for high need individuals experiencing a critical transition in care.
Individual awards will be $40,000 for the one-year contract. Funding may be used to cover program costs related to the development of a formal pathway with providers serving the target population, including but not limited to:
- Competitive salary and fringe for a dedicated Intake Specialist;
- Agency resources necessary to facilitate direct access to hospital/facility staff and information needed for coordinated discharge planning (e.g., agreements to access medical records, clearance to access inpatient unit, etc.);
- Staff time and agency resources necessary for participation in training;
- Transportation, equipment, and service documentation; and
- Allowance for flexible hours to optimize the CMA’s ability to communicate with inpatient team for coordination of discharges that occur outside normal business hours.
