HUD Issues NOFO for Older Adults Home Modification Grant Program
On September 20th, the U.S. Department of Housing and Urban Development (HUD) issued a Notice of Funding Opportunity (NOFO) seeking applicants to support implementation of the Older Adults Home Modification Program (OAHMP). This program enables low-income elderly persons to remain in their homes and avoid assisted care facilities by implementing low-cost, high-impact home modifications that reduce the risk of falls, improve general safety, increase accessibility, and support functional abilities in the home. Examples of home modifications include the installation of grab bars, railings, lever-handled doorknobs and faucets, temporary ramps, tub/shower benches, and raised toilet seats.
HUD will provide $30 million in total funding across 15 awardees through this NOFO, with $10 million specifically allocated to support rural populations. Individual grants will range from $1 million to $2 million. Contracts will last for three years, beginning January 15, 2025. Eligible applicants include not-for-profit 501(c)(3) organizations with at least three years of experience providing services to older adults and conducting home modifications.

OMH Issues RFP for the Reallocation of an Existing Apartment Treatment Program in NYC
On September 25th, the New York State (NYS) Office of Mental Health (OMH) issued a Request for Proposals (RFP) for the operation of an existing Apartment Treatment Program in Manhattan and the Bronx with capacity to serve 79 individuals in scattered site apartment units. The successful applicant will be required to transition the existing residents and leases from the current operator of the program. All housing units target adults with serious mental illness or co-occurring serious mental illness and substance use disorder.
Eligible applicants are not-for-profit 501(c)(3) organizations that are currently operating an OMH-licensed Apartment Treatment Program in New York City (NYC). Agencies with prior takeover experience and/or those with a proven track record of accepting OMH priority populations and transitioning individuals to more independent housing are encouraged to apply. Contracts will last for five years, starting on July 1, 2025.
Ongoing funding to support the operation of the Apartment Treatment units will be provided by a combination of Social Security Income (SSI) and Medicaid funding. The agency’s current programmatic structure will determine specific funding amounts. In addition, a partial Program Development Grant (PDF) up to $3,000 per bed will be available to assist with transitioning the program and establishing new units, as needed to fill vacancies.

DOH Seeks FQHC Participants to Implement Evidence-Based Interventions to Improve Breast and Cervical Cancer Screening
On September 16th, the NYS Department of Health (DOH) issued a Request for Bids (RFB) seeking Federally Qualified Health Centers (FQHCs) to participate in a project to increase breast and cervical cancer screening rates in primary care practice sites. The project intent is to develop and implement a quality improvement (QI) plan that includes at least two of the priority evidence-based interventions and other systems changes in each clinic site to make sustainable improvements in breast and cervical cancer screening rates. DOH will provide technical assistance on the development and implementation of an approved QI plan and will support learning opportunities for participating clinics.
DOH will provide up to $35,500 to each participating clinic over the 2.5-year contract period. Payments will be based on the achievement of deliverables within specified timeframes, as noted in Table 1 of the RFB. Contracts are expected to begin on January 1, 2025. To apply, an FQHC must identify a clinic site in which to implement the project that meets all of the following criteria:
- An FQHC clinic or look-alike that provides primary care services to the eligible populations for breast and cervical cancer screening;
- A current participating provider in the DOH Cancer Services Program;
- A clinic site with a breast cancer screening rate of less than 57.5% and a cervical cancer screening rate of less than 59.7%;
- Capacity to provide all required project data; and
- Staff capacity to support and participate in all project activities.
