RFA for Nursing Home Diversion and Traumatic Brain Injury Program Administration

On January 7th, the New York State Department of Health (DOH) reissued a Request for Applications (RFA) for the combined administration of Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) services in New York City. Over $7.9 million in total funding will be available through this RFA for the combined administration of NHTD and TBI services through a Regional Resource Development Center (RRDC) to be operated by a not-for-profit agency in New York City.

The NHTD and TBI waiver programs are both 1915(c) Home and Community Based Services (HCBS) waiver models that are designed to reduce the incidence of unnecessary institutionalizations.

The NHTD program is currently approved to serve up to 3,600 eligible seniors (over 65 years old) and/or adults (18 to 64 years old) with a physical disability. The TBI program is currently approved to serve up to 5,132 adults (18 to 64 years old) with a TBI diagnosis. RRDCs provide waiver operational and administrative functions on behalf of DOH.

The full RFA and a summary of major requirements are attached. Applications are due on February 13th.
