DOH Issues RFA for Administration of Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Waiver Programs
On February 12th, the New York State (NYS) Department of Health (DOH) issued a Request for Applications (RFA) for the local administration of Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) services. NHTD and TBI services are home and community-based services authorized under a 1915(c) waiver. Services provide a community-based alternative for eligible adults who require an institutional level of care but have chosen to receive care in their homes.
 
Through this opportunity, DOH will award approximately $13 million in total annual funding during the five-year contract period to qualified not-for-profit agencies that serve as Regional Resource Development Centers (RRDC) and assist DOH with administration of the NHTD and TBI waiver programs. Individual awards will range from $1.08 million to $3 million in annual funding, dependent on the service area. Applicants may apply to manage one or more of the service areas outlined in the RFA; however, separate applications will be required for each region to which the applicant is applying. DOH may also choose to award a single statewide contract covering all counties.
 
Eligible applicants are not-for-profit organizations with at least three years of experience providing and/or overseeing services for seniors or people with disabilities. Applicants cannot be a provider of waiver services, including a sub-corporation, foundation, or any other legal entity under the control of a waiver service provider agency. Awarded RRDCs will be responsible for performing operational and administrative functions on behalf of DOH, including but not limited to informing the community about waiver services, overseeing participant enrollment, addressing complaints, recruiting providers, and conducting training and technical assistance.

DOH Issues RFA for Community Aging in Place Initiative 
On February 20th, DOH issued an RFA to support implementation of the Community Aging in Place – Advancing Better Living for Elders (CAPABLE) pilot initiative. This initiative aims to improve the health and functioning of older adults with low income, enabling them to age in place safely and independently. Components include nursing, occupational therapy, and home modifications. DOH will provide $6.3 million in total annual funding to qualified organizations that implement and manage the CAPABLE initiative within their communities. There will be up to 20 awards across four regions, with each awardee receiving up to $315,000 annually during the three-year contract period.
 
Eligible applicants are not-for-profit 501(c)(3) organizations or for-profit organizations that provide or arrange services for aging individuals with functional needs or chronic health conditions. Applicants may submit one application per region outlined in the RFA.
