Health Home Supportive Housing RFA – Staten Island

On June 11th, the New York State Department of Health, in collaboration with the Office of Health Insurance Programs and the Medicaid Redesign Team, released a Request for Applications (RFA) for the Health Home Supportive Housing Program (HHSHP) in Richmond County (Staten Island). The HHSHP is a rental subsidy and transitional housing support services program for high-cost, high-need Medicaid members. This RFA will consolidate the funding for this program in Staten Island, which currently flows to two organizations (Project Hospitality and SI Behavioral Network), into one award. As of January 2020, the awarded applicant will receive $800,000 per year over four years, through December 2023, to provide rental subsidies and non-medical services to homeless Medicaid members in Staten Island who are enrolled in or in the process of enrolling in a Health Home, which may include both single individuals and families. Applicants must be able to serve the existing 42 applicants and continue to expand HHSHP on Staten Island.
