On May 8th, the New York State Department of Health (DOH) released a Questions and Answers document concerning the MRT Initiative requiring hospitals to refer eligible Medicaid-enrolled patients to Health Homes. This Questions and Answers document serves as a follow-up to the original DOH notification sent to hospitals on February 15th. As specified in the original notification, hospitals must develop and maintain policies and procedures to refer to Health Homes any Medicaid-enrolled patients who are eligible and submit a Letter of Attestation to DOH which confirms that such policies have been developed and implemented. The letter (available here) should be signed by the hospital’s CEO and submitted through the DOH website here by May 15th.
